From the paper’s Abstract:
The originators of LOPA have always been reluctant to use conditional modifiers (CM), enabling events (EE), and conditional severities (CS) in LOPA. This is not from lack of experience in LOPA or CM/EE/CS. The hesitation to use these factors is because unlikely Independent Protection Layers (IPLs) and Initiating Events (IEs), these extra risk reduction factors cannot be periodically validated and because changes that effect these factors are usually not managed/recognized. The authors share their experience on when using such factors are appropriate and what limits of risk reduction as appropriate.
